Delprojekterne skrider frem som planlagt

After seven months of dedicated effort, Nanna Selmer successfully completed the targeted 320 interviews with hospitalised medical patients – along with many meaningful conversations.

Nanna is now busy entering data and conducting follow-ups, so that the final prediction model can be developed. This model will identify elderly multimorbid patients at risk of readmission within 30 days.

Over the past six months, Matilde has recruited and interviewed 20 elderly patients with cardiovascular and multimorbid conditions about their experiences of hospitalisation and the internal transitions that accompany it.

Patienterne blev interviewet over telefonen, under indlæggelse og i eget hjem.

Matilde comments: “The patients’ perspective is very close to my heart, and I truly believe we have much to learn from their experiences and reflections. I’m looking forward to delving into the material and, in time, sharing some insightful and important results that will both deepen our understanding of internal transitions in hospitals and contribute to the development of the transition component of the project.”
