Bridging Safe Elderly Care deltager på netværkskonferencen ‘Partnerskaber for sundhed’

Innovation, technology and partnerships in healthcare** was the headline of today’s networking conference *“Partnerships for Health”*, organised by my talented colleagues from the Research Unit at Zealand University Hospital’s Staff Department.
In addition to a range of exciting presentations, project partners from **Zealand University Hospital and IQVIA presented their Interreg Öresund-Kattegat-Skagerrak project, Bridging Safe Elderly Care.

👉 Reducing the number of stressful and unnecessary readmissions among elderly patients with multimorbidity:
Using artificial intelligence, a data model is being developed to identify older multimorbid patients at high risk of readmission. This makes it possible to initiate interventions at home or in municipal care to prevent readmissions – benefiting patients, their relatives, and the entire healthcare system.

👉At forbedre håndteringen af ældre multisyges overgange mellem og på hospitaler: Vigtige informationer kan gå tabt ved flytninger og projektet vil sikre ét samlet overblik til personalet over alle relevante patientinformationer. Med brug af AI udvikles en prototype af en digital løsning, som kan integreres i Sundhedsplatformen, for dermed at sikre patienterne et trygt sygdomsforløb.
