Forskning kræver benarbejde

🔍 Research takes legwork – and that’s something we know well in Bridging Safe Elderly Care.
At a recently held general assembly, the spotlight was on a shared mission: creating safer transitions for elderly patients with multiple chronic conditions and reducing unnecessary readmissions.
Here’s a glimpse of what the project is currently working on:

👨‍💻 In Region Skåne, teams are working with machine learning and data sets to identify palliative patients and so-called ’frequent flyers’ – those who are readmitted most often.

🏥 At Zealand University Hospital, in collaboration with IQVIA, a prediction model is being developed to identify elderly multimorbid patients at risk of readmission. This has required the collection of large amounts of clinical patient data. Over seven months, 320 patients have been interviewed.

🤓 At IQVIA, the focus is on IT architecture, Python-based microservices, the AI engine, and the search for the right data.

👩‍⚕️ Which patient information is most important to pass on when a patient is transferred between departments or hospitals? We are asking healthcare professionals, patients and relatives that very question – and pointing to specific opportunities for improvement.

🧐 Together with nurses, we are also looking more closely at the role of discharge coordinators – and how their responsibilities can be strengthened.

🥅 The Research Department provides project management, financial oversight, and communication support – ensuring the project stays on track and reaches its goals.

 

There’s a long way from the general assembly to practical implementation – but we’re getting there, building bridges all the way.
