At this year’s National Conference for Nurses 👩⚕️👨⚕️ working in research and development, the big theme was collaboration – how to strengthen the transitions between sectors to ensure safety, continuity and reassurance for patients.
👉 Technology plays a key role in this shift, and Nanna Selmer contributed with insights from the Bridging Safe Elderly Care project.
She presented the development of a prediction model designed to identify older patients 👴🧓 with multiple conditions who are at high risk of being readmitted within 30 days.
“By using data to spot patients who need early intervention, we can sharpen our research, refine our strategies and make sure we act where it really makes a difference,” says Nanna Selmer.
