How can we improve internal transitions for elderly patients with cardiovascular and multiple chronic conditions during hospitalisation?
This was one of the questions discussed at the workshop hosted by the Cardiology Department at Zealand University Hospital, Roskilde.
Nurse and PhD student Matilde Skødstrup Axelsen has, over three months of observations, examined nurses’ workflows, communication, and documentation when patients are transferred during internal transitions between units, departments, and hospitals.
The aim of the day’s workshop was to share preliminary findings and gather input from the department’s nurses and a representative from the Danish Society for Patient Safety.
Openness and engagement resulted in many valuable contributions, bringing the project closer to the next step: developing a prototype for a digital transition page. The goal is to create a solution that both ensures crucial information for patients and simplifies documentation for staff during internal patient transitions.
