class=\"partner__excerpt\">\n\t\t\t\t\t\t\t\tParticipating from Zealand University Hospital are the Department of Medicine in K\u00f8ge, the Department of Cardiology in Roskilde, and the Research Department.\n\t\t\t\t\t\t\t<\/p>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/button>\n\n\t\t\t\t\t<div class=\"partner__wrapper\">\n\t\t\t\t\t\t<div class=\"partner__text\">\n\t\t\t\t\t\t\t<p>Region Zealand brings extensive experience: <a href=\"https:\/\/www.sjaellandsuniversitetshospital.dk\/afdelinger\/medicinsk-afdeling-koege\" target=\"_blank\" rel=\"noopener\">The Department of Medicine in K\u00f8ge<\/a> has\u00a0gained significant knowledge through a new working method focusing on the discharge of elderly patients with multiple conditions, which is utilised in this project.<\/p>\n<p>The patient and healthcare professional perspective on transfers within a single department or between departments at the hospital is covered by <a href=\"https:\/\/www.sjaellandsuniversitetshospital.dk\/afdelinger\/kardiologisk-afdeling\" target=\"_blank\" rel=\"noopener\">The Department of Cardiologi in Roskilde<\/a>.<\/p>\n<p>Both the Department of Medicine in K\u00f8ge and the Department of Cardiology in Roskilde are affiliated with the Institute of Regional Health Research at the University of Southern Denmark.<\/p>\n<p><a href=\"https:\/\/forskning.eu\/forskningsenheden\/\" target=\"_blank\" rel=\"noopener\">The Research Department<\/a> is responsible for project management, communication, and collaboration with the other project partners, IQVIA will develop a digital solution for patient record systems in Denmark and Sweden.\n\t\t\t\t\t\t\t<\/p>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/button>\n\n\t\t\t\t\t<div class=\"partner__wrapper\">\n\t\t\t\t\t\t<div class=\"partner__text\">\n\t\t\t\t\t\t\t<p>In collaboration with the other project partners, <a href=\"http:\/\/www.iqvia.com\" target=\"_blank\" rel=\"noopener\">IQVIA<\/a> will develop a digital solution for patient record systems in Denmark and Sweden.<\/p>\n<p>IQVIA has extensive experience in healthcare innovation and the implementation of digital initiatives. Together with the project&#8217;s other partners, they will develop a digital \u2018add-on\u2019 solution for the patient record systems in Denmark (Sundhedsplatformen) and Sweden (Sk\u00e5nes Digitale V\u00e5rdsystem). The solution will ensure easy access to relevant patient data for healthcare staff and make it possible to identify which patients with multiple conditions are expected to have high healthcare needs.<\/p>\n\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<div class=\"partner\">\n\t\t\t\t\t<button class=\"partner__header\">\n\t\t\t\t\t\t<div Region Sk\u00e5ne, the project&#8217;s primary partner is the Geriatrics Department at Helsingborg Hospital, along with project leaders and participants from the care sector in Northwest Sk\u00e5ne.\n\t\t\t\t\t\t\t<\/p>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/button>\n\n\t\t\t\t\t<div class=\"partner__wrapper\">\n\t\t\t\t\t\t<div class=\"partner__text\">\n\t\t\t\t\t\t\t<p>They contribute specialised expertise and experience with elderly patients with multiple conditions, including from a mobile geriatric team and cooperation with municipalities in elderly care.<\/p>\n<p>Link patient-focused healthcare technology.\n\t\t\t\t\t\t\t<\/p>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/button>\n\n\t\t\t\t\t<div class=\"partner__wrapper\">\n\t\t\t\t\t\t<div class=\"partner__text\">\n\t\t\t\t\t\t\t<p><a href=\"https:\/\/www.cec.lu.se\/research\/research-themes\/computational-science-health-and-environment\" target=\"_blank\" rel=\"noopener\">Lunds Universitets COSHE<\/a> will also explore the potential to identify which patients with multiple conditions are expected to have high healthcare needs in the future.<\/p>\n<p>Link to <a
