At this year\u2019s National Conference for Nurses \ud83d\udc69\u200d\u2695\ufe0f\ud83d\udc68\u200d\u2695\ufe0f working in research and development, the big theme was collaboration \u2013 how to strengthen the transitions between sectors to ensure safety, continuity and reassurance for patients.<\/p>\n
\ud83d\udc49 Technology plays a key role in this shift, and Nanna Selmer contributed with insights from the Bridging Safe Elderly Care project.
She presented the development of a prediction model designed to identify older patients \ud83d\udc74\ud83e\uddd3 with multiple conditions who are at high risk of being readmitted within 30 days.<\/p>\n
