Region Zealand brings extensive experience: The Department of Medicine in Køge has gained significant knowledge through a new working method focusing on the discharge of elderly patients with multiple conditions, which is utilised in this project.

The patient and healthcare professional perspective on transfers within a single department or between departments at the hospital is covered by The Department of Cardiologi in Roskilde.

Both the Department of Medicine in Køge and the Department of Cardiology in Roskilde are affiliated with the Institute of Regional Health Research at the University of Southern Denmark.

The Research Department is responsible for project management, communication, and finance.

In collaboration with the other project partners, IQVIA will develop a digital solution for patient record systems in Denmark and Sweden.

IQVIA has extensive experience in healthcare innovation and the implementation of digital initiatives. Together with the project’s other partners, they will develop a digital ‘add-on’ solution for the patient record systems in Denmark (Sundhedsplatformen) and Sweden (Skånes Digitale Vårdsystem). The solution will ensure easy access to relevant patient data for healthcare staff and make it possible to identify which patients with multiple conditions are expected to have high healthcare needs.

They contribute specialised expertise and experience with elderly patients with multiple conditions, including from a mobile geriatric team and cooperation with municipalities in elderly care.

Lunds Universitets COSHE will also explore the potential to identify which patients with multiple conditions are expected to have high healthcare needs in the future.
